Laceration of atrial septum during balloon sizing of atrial septal defect.

چکیده
We read with great interest the recently published case report by Alsaileek et al. in the European Journal of Echocardiography on laceration of atrial septum during balloon sizing. We had already reported about tear of atrial septum during balloon sizing of atrial septal defect (ASD) in a 20year-old female. Balloon sizing was attempted with Amplatzer sizing balloon (Amplatzer sizing Balloon, AGA medical corporation, 682 Mendelssohn Avenue, Golden Valley, MN-55427) (length, 24 mm; maximum volume, 30 ml). At partial inflation a good indentation was obtained which slowly disappeared without further increase in the balloon volume. Trans-thoracic echocardiography (TTE) followed by trans-esophageal echocardiography (TEE) revealed a tear in the inferior margin of the septal defect. The patient was referred for surgical closure where a tear in the inferior rim of the ASD was found which extended down the septum up to the opening of inferior vena cava, with a small hematoma. Patient underwent repair of the tear and pericardial patch closure of the defect. In our patient we suspected the tear because the waist disappeared over time, which was confirmed by TTE. In the case reported in this journal echogenic shadows were noted at atrial septum during the procedure which was confirmed as laceration at surgery. This laceration involved the superior and posterior aspects of the atrial septum, while our patient the tear extended from the inferior rim of the defect. Alsaileek et al. used Meditech balloon for sizing the defect employing the pull-through technique. It is not clear whether Alsaileek et al. used a fluoroscopy-guided or TEE-guided technique for ASD sizing. The procedure in our case was

Transcatheter closure of atrial septal defect (ASD) has become an accepted alternative to surgery. A number of complications associated with ASD device closure have been recognized but most are rare or minor in severity. We report a rare complication of atrial septal laceration during transcatheter closure of secundum ASD. We discuss the diagnostic confusion, which resulted in the decision for ...
متن کاملIntegrated Percutaneous Atrial Septal Defect Occlusion and Pulmonary Balloon Valvuloplasty
Introduction: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases during childhood. Today, ASD closure is done by occlutech device via cardiac catheterization. ASD repair with transcatheter technique has shown high closure rate. However, coexistence of severe pulmonary stenosis and large defect in atrial septum is rare. When these two problems coexist, the result is i...
متن کاملTranscatheter closure of atrial septal defects--is balloon sizing still necessary?
INTRODUCTION The device closure of atrial septal defects has evolved over the years. In the early days of transcatheter occlusion, balloon sizing was used to choose an appropriate sized device. We postulate that balloon sizing does not value-add to the procedure and is unnecessary. MATERIALS AND METHODS Patients who had balloon sizing, with (Group 1, n = 38) or without (Group 2, n = 21) atria...
متن کاملTranscatheter atrial septal defect closure in an infant (body weight 6.4 kg) using the GORE CARDIOFORM septal occluder (GCSO)
INTRODUCTION Transcatheter closure has become the treatment of choice for secundum atrial septal defects (ASD II), but particularly in small children, there is concern regarding procedure-related complications. CASE DESCRIPTION We report on a 10-month-old infant, body weight of 6.4 kg, with a large ASD who was referred for failure to thrive and dyspnea on exertion. Echocardiography showed two...
متن کاملFunctional and anatomical correlates in atrial septal defect. An echocardiographic analysis.
The results of cross sectional echocardiography, intracardiac contrast echocardiography, and balloon sizing techniques and conventional haemodynamic assessment were correlated in 40 consecutive patients evaluated for an isolated left to right shunt at atrial level. Echo free areas along the septum were identified in 23 of 25 patients with a secundum defect, but not in two with a fenestrated def...
